Advanced Practice Provider (Nurse Practitioner)
About The Position
Town Square Health is hiring a Primary Care Provider (NP) in the Rochester area to deliver high-quality primary care in a Medicare-focused, value-based care environment. This is a clinic-based role with the time, team support, and resources to practice comprehensive medicine: patient panels smaller than the national average, 20–40-minute visit lengths based on patient needs, and a team-based care model with two Medical Assistants paired with each provider. You will play a key role in delivering excellent clinical care, coordinating across the care team, and helping shape a new care model designed to improve outcomes and patient experience.
Requirements
- Nurse Practitioner (NP), DNP or equivalent experience strongly preferred.
- National certification in at least one of the following specialties: Family Nurse Practitioner Adult-Gerontology Primary Care Nurse Practitioner Adult Nurse Practitioner Gerontological Nurse Practitioner
- 2-3 years minimum experience as an NP
- Active New York State Nurse Practitioner licensure in good standing or must obtain prior to start date
- Applied for DEA registration prior to hire/start date
- Must meet all applicable employer and payer credentialing requirements
- Ability to meet post-offer requirements, including drug screen and health assessment
- Compliance with all New York State health requirements
