About The Position

Advanced Practice Provider (APP)/ Nurse Practitioner (NP)/Physician Associate (PA) POSITION SUMMARY Obtain medical history and preform physical examinations Perform c linical assessments and develop treatment plan Initiate, monitor, and adjust medications in accordance with the APP Practice Agreement Manage patients with complex cardi ovascular diseases Order and evaluate laboratory, diagnostic and radiographic tests Complete medical records and billing at the end of each day Coordinate care with rounding physician or office based physician Provide education to patients and their family members Provide pre and post procedural education for patients scheduled for cardiac testing Coordinate care between other physicians, nursing staff, and interdisciplinary providers Make appropriate referrals to community and other interdisciplinary providers Refer to appropriate Virginia Heart Programs: Heart Failure Program, Risk Factor Modification Program, Research Program , Ideal Protein Program Act as liaison for the physician in communicating with patients and family members Hospital Specific : Perform daily hospital rounds , coordinate daily work with the rounding MDs at the beginning of day, prioritizing patient care based on severity of illness, new patient consults/admissions and providing timely discharges. Maintain communication with the rounding physicians. C ommunicate unresolved issues to the MD at the end of the day . Maintain hospital list and send in billing and scheduling sheets at the end of the day . Coordinate scheduling of inpatient stress test, cardiac cath , and other inpatient and outpatient testing. Provide discharge education and outpatient follow up plan O ffice Specific : Maintains own schedule of appx 15 patients in a 9-hour day. Physician is available on site for consultation and collaboration as needed. Types of patient visits: Pre op risk assessments, hospital follow ups, Heart Failure Clinic, Risk Factor Modification, Symptom calls, post cardiac testing visits to review result s, hypertension F/U, urgent add- ons/walk - ins, and all other routine cardiology follow ups. Requirements

  • Graduate of an accredited Nurse Practitioner Program or Physician Associate Program (Master’s level of preparation)
  • NP Credentialed by the ANCC (American Nurses Credentialing Center)
  • PA must be Board Certified
  • BLS/CPR and ACLS certified
  • Licensed in State of Virginia
  • Have appropriate hospital privileges
  • Attend CME yearly

Nice To Haves

  • Experience in Cardiology recommended– preferably as an APP
