Be a part of a world-class academic healthcare system University of Chicago Medicine as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for our Hematology/Oncology group at Silver Cross Hospital. The Advanced Practice Provider (APP) will provide comprehensive care to oncology patients within the Section of Hematology/Oncology in The Department of Medicine at the Silver Cross Hospital location. The APP will focus on patients receiving care in the outpatient setting (IV Therapy or Clinic) within our oncology program. The APP will assess and manage side effects from chemotherapy, radiation and immunotherapy. The APP will work collaboratively with oncology attendings, fellows and nurses and be proficient to independently recognize signs and symptoms of disease progression or response to therapy. Ideal candidate will be familiar with the treatment of malignancy with standard of care regimens as well as on clinical trials. Unit-specific Responsibilities The Advanced Practice Provider will provide comprehensive care to patients for the Section of Hematology/Oncology in The Department of Medicine at the Silver Cross Hospital location. Work collaboratively within a multidisciplinary team, using advanced clinical skills to manage a caseload of patients. Assess, diagnose, initiate delegated medical treatments, perform independent nursing interventions and evaluate care. Provide a model of care that is shaped by theory, expanded by science, research and reflective practice and is identified by a holistic and comprehensive approach to the patients. Staff half-day or full-day clinics as assigned; attend multidisciplinary conferences of assigned disease area. Manage the care of patients, including assessment, diagnosis and treatment for assigned patients. Document history and physical examination and changes in management according to government guidelines in the outpatient clinic at Silver Cross Hospital. Coordinate diagnostic tests, follow-up on tests, and other patient care issues. As part of duties, perform reporting for government agencies, chart review, refill prescriptions, and answer phone calls for patients for whom the APP is involved with longitudinal care. Attend conferences and yearly continuing education courses to keep abreast of hematology/oncology patient management.
