The Full Time APP - Geriatrics position at the Pace Clinic involves collaboration with a physician to expand individualized primary healthcare services for patients, particularly those aged 55 and older. The role includes diagnosing and treating acute or chronic health conditions, performing health assessments, ordering and interpreting diagnostic studies, and providing direct treatment and management of health conditions. The APP will work mostly independently under the direction of a supervising physician, with a call schedule of 1:6, currently at 1:4. The position requires seeing 4-6 patients daily in a clinic setting, with a focus on geriatric care. Candidates should have at least 3 years of primary care clinic experience, strong critical thinking skills, and a willingness to learn new medical knowledge.

  • Obtain patient history and perform assessment via observation, interview, and examination.
  • Order, perform, and interpret diagnostic studies.
  • Perform preventative health assessments, screening, immunizations, and patient care.
  • Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
  • Counsel and educate patients and their families/caregivers concerning preventative health, treatment options, and community resources.
  • Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.
  • BLS Provider obtained prior to hire date or job transfer date required.
  • Nurse Practitioner obtained prior to hire date or job transfer date required.
  • Registered Nurse obtained prior to hire date or job transfer date required.
  • Master's degree of Nursing required.
  • Geriatric experience is a plus.
  • Experience in a geriatric setting, palliative care, or hospice setting would be a great asset.
