About The Position

Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (APP) for the Comprehensive Home and Palliative Care Team in Columbus, OH. The APP will collaborate with physicians to coordinate healthcare for patients in their homes, monitor patient status, and implement treatment plans to improve care outcomes. This role involves working Monday to Friday, focusing on patient assessments, care planning, and education.

Requirements

• Master's Level Degree or equivalent combination of education and experience.
• Must be a Certified Nurse Practitioner (CNP) or Physician Assistant (PA) by a nationally recognized board in the state of Ohio.
• Experience working in a primary care setting or palliative/hospice as a CNP or PA preferred.
• Proficient in clinical practice protocols and delivering primary care.
• Extensive knowledge of medical terminology.

Nice To Haves

• EPIC experience is helpful.
• Knowledge and understanding of Palliative and Hospice care.

Responsibilities

• Develop patient care treatment plans by obtaining medical histories and examining patients during appointments.
• Coordinate care planning with primary care physicians, specialists, and other care team members.
• Facilitate goals of care conversations and support a palliative care approach when indicated.
• Perform routine or annual physical examinations.
• Treat or refer patients for primary care conditions as necessary.
• Educate patients and caregivers on conditions, outcomes, or treatment plans.
• Consult with physicians and clinical staff regarding treatment plans and patient progress.
• Prescribe medication dosages, routes, and frequencies based on individual patient needs.
• Order, perform, and/or interpret results of diagnostic tests when necessary.
• Participate in monitoring patient outcomes and assure quality of care by adhering to professional standards.

• Full-time position with a competitive salary range of $86K - $109K per year.
