Cleveland Clinic’s Integrated Hospital Care Institute is currently seeking a motivated and qualified Physician Assistant, Clinical Nurse Specialist, or Nurse Practitioner (Adult or Family certified) for a full-time position with the Pulmonary Vascular Program. This provider will oversee the care of adult patients with pulmonary hypertension (PH). The PH program is a model collaborative practice with an interprofessional team that affords exciting opportunities to participate in transitions of care, managing complex care needs, research on a local and national scale, and the potential to become a leading expert in pulmonary vascular disease medicine. The patient population will include Adults only. The hours for this position are Monday-Friday with no weekends, holidays or call. This position is primarily outpatient but includes some collaboration with main campus inpatient service for admitted PH patients. New graduates are welcome to apply. Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs. APRN: Nationally certified licensed healthcare provider who provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings. Functions in collaboration with a physician according to a standard care arrangement and specialty certification. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. PA: Nationally certified licensed healthcare provider who provides primary and or specialty medical care (the practice of medicine) in a variety of healthcare settings. Functions the collaboration agreement of a Physician, (as per O.R.C. 4730) performs complete histories and physicals on patients of all ages to establish and record patient's current health status and to develop a working diagnosis and treatment plan to provide continuing medical care, including daily examinations, writing orders, chart notes, discharging patients and completing discharge summaries. This medical care may be in a variety of clinical settings. PA's are privileged by the Board of Governors. PA's in the Hospital care role provide outpatient and/or hospital based care to patients (more than 50% of their time) with complex, acute, and chronic health conditions. Outpatient/Hospital care PA's practice in a setting that requires the performance of diagnostic and therapeutic interventions to manage acute and chronic health problems. Must have the ability to work with multi-disciplinary teams.
