Cleveland Clinic is seeking a passionate and patient-focused Advanced Practice Provider (Family Nurse Practitioner or Physician Assistant) to join the Family Medicine team at Cleveland Clinic Union Hospital Sugarcreek. This full-time opportunity is ideal for an APP who values relationship-based care, clinical excellence, and working within a highly collaborative, supportive environment. In this role, the APP will provide comprehensive primary care services to patients across the lifespan, delivering high-quality, evidence-based care focused on health promotion, disease prevention, and the diagnosis and management of acute and chronic conditions. Responsibilities include conducting thorough patient assessments, developing and implementing individualized treatment plans, prescribing medications, coordinating care across specialties, and educating patients to support long-term wellness and self-management. The APP will work closely with physicians, nursing staff, and interdisciplinary team members to ensure seamless continuity of care and outstanding patient experiences—fully aligned with Cleveland Clinic’s mission of putting patients first. Nationally certified Advanced Practice Provider who provides primary and/or specialty care in a variety of healthcare settings. Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs.
