About The Position

This is a great opportunity for the right candidate to provide medical services to Ascension Texas Cardiovascular patients in Kyle, TX. As an APRN or PA in Cardiology with Ascension, you will provide care to patients with congestive heart failure in an outpatient clinic. The APP will also help manage the heart failure patient population at Ascension Seton Hays with one of the Advanced Heart Failure cardiologists. This role aims to provide quality heart failure care and help reduce hospital readmission for a complex patient population. You will collaborate with cardiologists and other team members, coordinate with other specialties involved in each patient's care, and provide a continuum of care.

Requirements

• Nurse Practitioner credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
• Licensed Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.
• Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.
• Master's degree of Nursing required.

Nice To Haves

• Cardiology experience is preferred.

Responsibilities

• Obtains patient history and performs assessment via observation, interview and examination.
• Orders, performs and interprets diagnostic studies.
• Provides direct treatment and management of health conditions via referral to other healthcare providers or community resources.
• Counsels and educates patients and their families/caregivers concerning treatment options and community resources.
• Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.
