About The Position

The Advanced Practice Provider is responsible for delivering primary care services with a focus on Women's Health and Obesity. This role involves assessing, diagnosing, prescribing, treating, and educating patients while collaborating closely with physicians and other clinicians in a team-oriented environment. The position is full-time and benefits eligible, operating within a clinic that is open Monday to Friday from 8 am to 5 pm.

Requirements

  • Successful completion of advanced practitioner training and certification from an approved nurse practitioner or physician assistant program.
  • Current CPR certification.
  • Graduate of an accredited Physician Assistant or Nurse Practitioner program.
  • NCCPA certification and Minnesota Board of Medical Practice registration/license.
  • DEA registration.
  • Experience in family/internal medicine preferred.
  • Experience with chronic diseases such as hypertension, diabetes, and hyperlipidemia preferred.
  • Certificate of Advanced Education in Obesity Medicine preferred.
  • At least 3 years of primary care NP experience and certification in Obesity Management.

Responsibilities

  • Assess patient status by obtaining health history through patient/family interviews and chart reviews.
  • Perform appropriate physical examinations and order diagnostic tests.
  • Integrate data to determine diagnosis and therapeutic plans, identifying health risks.
  • Develop and implement treatment plans by prescribing medications and handling minor procedures.
  • Educate patients and families regarding medications, treatments, and health promotion.
  • Communicate case management information to other professionals and community agencies.
  • Prepare documentation for medical records, updating patient charts with examination results and treatment plans.
  • Participate in peer reviews, staff education, and quality assurance activities.
  • Act as a consultant to the clinical team in the area of specialty.
  • Promote patient advocacy and ensure compliance with confidentiality requirements.
