Comprehensive Neuroscience patient management: Clinical Support for the CHS Neuroscience Service Line Initiatives in collaboration with CHSLI affiliated Provider Groups Job Details Responsibilities: All phases of Neuroscience patient management including but not limited to: Provide clinically applicable evidence based practice of the hospitalized patient inclusive of the pre, intra & post-operative care period to the Neuroscience patient population. Medical management of the Neuroscience population in collaboration with other medical disciplines to ensure patient throughput across the CHS care continuum. Perform Neuroscience Consultations & perform clinical responsibilities as delegated by the Service Line leadership. Render patient specific care including ordering, reviewing and interpreting laboratory & radiologic studies. Comprehensive clinical documentation in the EMR in compliance with Neuroscience coding initiatives. Educate patients, families & care providers with disease processes & implemented plan of care to ensure high levels of patient satisfaction. Discharge planning education including review of discharge instructions and medication prescribing in compliance with state & federal regulations. Coordination patient care plans with Consulting Physicians and other clinical providers involved in the management of Neuroscience patients. Maintain ongoing timely reports of clinical duties as required by CHS compliance initiatives. Facilitate patient transfers to appropriate internal & external healthcare entities when appropriate. May provide surgical assisting services when need arises under direction of service line leadership. Provide professional development direction and clinical education for peers and students within the Neuroscience Service Line.
