About The Position

The Advanced Practice Provider (APP) Lead is responsible for providing exemplary clinical care while also serving as a leader and mentor for advanced practice providers within Bluestone. This role encompasses clinical excellence, administrative responsibilities, and fostering interprofessional collaboration to ensure Advanced Practice Providers are supported sufficiently for their long term success at the organization. Expectations of this leadership role include ensuring the successful integration of new APPs into the practice, mentoring, and coaching APPs on best practices, reviewing performance metrics, and supporting clinical operations. New Hire APP Training and Support: Provide hands-on training for procedures and ad hoc support for clinical, operational, and practice management questions. Performance Coaching & Follow-Up: Provide mentorship and guidance to APPs on clinical decision-making, patient management, and workflow optimization. Committee Participation & Stakeholder Engagement: Represent APPs to ensure your perspectives and needs are represented in decisions. Community (ALF) Relationship Building and Stabilization: Support community-based initiatives and serve as a consistent presence during provider transitions to ensure standard workflows and optimal customer service. New Building & Program Launches: Attend ALF community launches to ensure operational standards are successfully implemented and demonstrate exemplary customer service. Field Coverage: While this role is not intended to provide extensive field support in the form of direct patient care, at times, the Lead may be asked to support the practice by providing intermittent field coverage as needed.

Requirements

  • Minimum 3 years experience as an Advanced Practice Provider in geriatrics or complex chronic disease management.
  • Must demonstrate a consistent record of professional conduct and performance that aligns with organizational values of dedication, excellence, collaboration, and caring
  • The employee must not have any active performance improvement plans (PIPs), disciplinary actions, or unresolved corrective measures
  • The employee should exemplify reliability, accountability, and a positive contribution to team dynamics and company culture.
  • Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
  • Board Certified/Board Prepared through AANP, ANCC, or NCCPA
  • Licensed through the Wisconsin Board of Nursing or Board of Medical Practice
  • DEA certification for the State of Wisconsin
  • Proven ability to communicate effectively with strong verbal skills
  • Excellent interpersonal and customer service skills
  • Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required
