About The Position

Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner or Physician's Assistant to join our Home-Based Primary Care Program in Baltimore, Maryland. This growing program focuses on high needs geriatric home-limited patients and provides high-touch, comprehensive primary care. Collaborating with the Medical Director and other team members, the Nurse Practitioner (NP) will provide medical care for a small panel of home bound and medically complex older adults. The NP will be responsible for evaluating and treating patients receiving primary care at home, coordinating their care with other team members and specialists, and participating in weekly inter-disciplinary team meetings. Requirements

  • NP Licensure in the State of Maryland required.
  • Must have a working vehicle to travel to patient's homes.
  • CPR Certification required.
  • 3 years experience required

Nice To Haves

  • Palliative and/or geriatric certification/experience preferred.
