About The Position

Internal Medicine APP Full-time House Calls Team - Intermountain Health Provo, UT Intermountain Health is an integrated, not-for-profit health system with hospitals and clinics throughout the Intermountain West. As an Advanced Practice Provider, you deliver high-quality primary care to some of our most vulnerable patients—often those with complex health needs—in the comfort of their homes. As a trusted clinical leader on the Intermountain House Calls team, you’ll play a pivotal role in shaping patient outcomes through a collaborative, multidisciplinary approach. Working alongside nurse care managers, social workers, and other dedicated professionals, you’ll ensure comprehensive, compassionate care that truly changes lives. To be successful in this role, the APP will need to be driven, able to self-direct, and have a passion for both learning and partnering with others to ensure care excellence. The APP will need to maintain flexibility in both coverage types and travel ranges (e.g., integrated support for sister programs and all-state coverage may occur), with reliable transportation (Intermountain-paid mileage). You will practice in the remote/ work-from-home setting, and support the patient with in-person visits to include video visits Your clinical hours will be: 700am – 530pm x 4 days/week, excluding weekends and holidays The On-Call schedule rotates quarterly, 1-2 weeks per quarter or more depending on staffing levels Competitive Compensation Incentives: Up to $7,500 relocation bonus (if applicable) Excellent opportunity for mentorship, leadership roles, program expansion, professional growth, and plenty of program support. Requirements

  • Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN).
  • Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
  • Eligible for State Controlled Substance License II through V.
  • Eligible for Federal DEA Certificate II through V.
  • BLS Certification for Healthcare Providers.
  • Other certification as required (e.g. ACLS, PALS, TNCC, NRP).

Nice To Haves

  • Master’s degree.
  • Education must be obtained from an accredited institution.
  • Degree will be verified.
