About The Position

Are you a dedicated Nurse Practitioner or Physician Assistant passionate about making a difference in patient care? Sentara Medical Group is actively seeking a compassionate healthcare professional to join our office-based Comprehensive Care team in Norfolk, VA. Join an integrated network of providers committed to creating a positive patient experience in an environment where everyone feels seen, heard, valued, and supported. Schedule: 7:30AM - 5:00PM 4 days per week patient-facing, with one half-day dedicated to administrative time (remainder of the day off). Provide comprehensive/primary care to chronically ill patients in an office setting. Average of 14 patients per day. Focus on reducing hospital readmission rates through proactive patient management and education. Conduct thorough assessments, diagnose conditions, and prescribe appropriate treatments. Robust APP onboarding program consistent with didactic curriculum, specialty shadowing, competency-based evaluations, and mentorship.

Requirements

• Active NP or PA.
• At least 5 years’ experience as APP.

Responsibilities

• Provide comprehensive/primary care to chronically ill patients in an office setting.
• Average of 14 patients per day.
