About The Position

We are hiring an experienced APP for our Mission Farms Primary Care Clinic in Overland Park, KS. This position is accountable for advanced nursing practice in the care of assigned patient populations Responsible for collecting and analyzing client health data Identifies appropriate expected outcomes Develops and implements patient plan of care Provides comprehensive clinical coordination of care and case management Provides consultation to influence plan of care for clients using prescriptive authority procedures and treatment in accordance with state and federal laws and regulations Evaluates clients progress in attaining expected outcomes Conducts nursing research and provides education to patient family and nursing staff Candidates with 2-3 years of advanced practice preferred.

• Applicable Experience: 1 year Advanced Practice Provider (KS) - Various, Advanced Practice Provider (MO) - Various
• Master's Degree
