We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,300 providers in Indianapolis, IN. This Advanced Practice Provider opportunity is located at our Community Heart and Vascular Hospital on our north campus. Community Physician Network is seeking an experienced Nurse Practitioner or Physician Assistant to work alongside our General Cardiology team at Community Heart and Vascular Hospital located in Indianapolis, IN. • Full-time, days • Weekend / call rotation is required. Requirements

Educational background consistent with requirements for licensure and certification within the state of Indiana
Experience in Advanced Heart Failure is strongly preferred
