About The Position

Dartmouth Health’s Department of Community and Family Medicine is seeking a Family Medicine APRN or PA to join our academic Primary Care practice at our Heater Road Clinic in Lebanon, NH. The ideal candidate would be a skilled clinician with at least 5 years of direct primary care provider experience. We are seeking a candidate who is an enthusiastic teacher and a scholar, committed to improving the health of our patients through high-quality team-based care, specifically in an academic setting. Our new team member would provide healthcare to individuals and families, assess and manage the health care needs and issues within specialty area, emphasizing health promotion and disease prevention as well as the diagnosis and treatment of acute and chronic illnesses. Function as part of the provider team to deliver the highest quality and safest patient care in a variety of settings, including community-based care. May assume primary responsibility for patient care. The Heater Rd. team is located in a beautiful outpatient facility three miles from Dartmouth-Hitchcock Medical Center. We are currently seeking candidates with 3+ years of primary care provider experience for this role. Some of this experience should be in a community care setting. The Family Medicine team at Heater Road is an integral part of a dynamic Primary Care group that has a focus on clinical and educational quality improvement, population health, and the development of new models of care delivery. We strive to provide team-based, high-value quality care to all patients within an academic setting where teaching, learning, and scholarship are central to our work and mission. Unique aspects of our practice include Family Medicine physicians who have additional training in Sports Medicine, Sleep Medicine, Functional Medicine, and Addiction Medicine, and we provide the potential for leadership development opportunities. Our integration within Dartmouth-Hitchcock Medical Center provides for unique collaborative relationships across an extensive array of specialties, as well as tremendous technical and administrative support.

Requirements

  • Graduate of an accredited APRN or PA academic program.
  • National specialty certification as FNP, WHNP, PNP, ANP or certification through the National Commission on Certification of Physician Assistants (NCCPA).
  • Eligible for RN licensure in the State of NH (APRN only).
  • Eligible for APRN or PA licensure in the State of NH.
  • Eligible for Federal DEA Registration.
  • BLS certification.
