About The Position

Intermountain Healthcare is seeking a Family Medicine Advanced Practice Provider (APP) for a full-time position at the West Valley Family Medicine Clinic in West Valley City, UT. Requirements

• One (1) or more years of experience preferred.
• Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN).
• Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
• Eligible for State Controlled Substance License II through V.
• Eligible for Federal DEA Certificate II through V.
• BLS Certification for Healthcare Providers.
• Other certification as required (e.g. ACLS, PALS, TNCC, NRP).

• Masters degree.
• Education must be obtained from an accredited institution.
