About The Position

Family Medicine APP (Family Medicine) Part-time (30 hours a week) Alta View - Intermountain Health Sandy, UT Intermountain Health is an integrated, not-for-profit health system with hospitals and clinics throughout Utah, as well as a hospital in Burley, Idaho. Great team dynamic New behavior health clinic set to open June 2026 New patients behavioral health will be seen within 24 hours of admission. A relevant history will be obtained, and a physical examination will be performed. Will formulate evidence-based, patient-centered treatment plans for chronic and acute episodic medical conditions. Will order and interpret tests, order medications, provide patient education, and recommend appropriate post-discharge referrals for medical conditions. Documentation of initial assessment and follow-up visits will be completed and signed on the day of service with the appropriate billing codes. Collaborate with the attending psychiatric provider and members of the interdisciplinary team as needed, including nursing staff, social work, pharmacists, and navigators. Will consult with hospitalists and other specialists as needed. Will assist with coordination for transfer to a higher level of care within the hospital when appropriate to ensure patients receive the appropriate level of care required for their medical conditions. Requirements

  • Two (2) or more years of experience preferred.
  • Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN).
  • Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
  • Eligible for State Controlled Substance License II through V.
  • Eligible for Federal DEA Certificate II through V.
  • BLS Certification for Healthcare Providers.
  • Other certification as required (e.g. ACLS, PALS, TNCC, NRP).

Nice To Haves

  • Masters degree.
  • Education must be obtained from an accredited institution.
  • Degree will be verified.
