Community Health Network has an excellent opportunity for a family medicine trained Advanced Practice Provider to work at our outpatient Washington, Family Care Medicine practice. Requirements

Family Medicine experience as an Advanced Practice Provider is preferred
Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.
Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
