About The Position

Franciscan Physician Network is seeking Diabetes and Endocrinology Advanced Practice Provider to join our group at services at Franciscan Health Diabetes & Endocrinology Specialists located in Lafayette, Indiana. Ideal candidate will have past or present endocrinology experience, strong focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care. Flexible full-time schedule; no weekends or holidays Clinic hours: Monday – Friday, 8:00 a.m. – 5:00 p.m. Average outpatient volume: 12-21 patients per day Extensive resources including onsite Clinical Diabetes Educators Potential outreach to surrounding locations Common Diagnoses: Type I Diabetes Type II Diabetes Insulin pumps Continuous Glucose Monitoring Primary Hyperparathyroidism Hyperthyroidism, Hypothyroidism Hypogonadism Polycystic Ovarian Syndrome Pituitary-Adrenal Disorders Franciscan Health is a non-profit health care ministry with primary and specialty care physician groups located throughout Indiana and Illinois. Requirements

• Master's Degree - Required
• Physician Assistant Certified (PA-C) – Required - or Nurse Practitioner (NP) or Nationally Recognized Certification - Required
• Basic Life Support Program (BLS) - American Heart Association - Required
