About The Position

The advanced practice provider (APP) provides clinical care to patients in a family-centered model with complex acute, critical and chronic health care conditions, advises patients on health maintenance and disease prevention, and/or provides skilled assistance in navigation of the health care system as appropriate to the developmental, cultural, emotional, and medical, surgical, or rehabilitative needs of the patient and their families. The APP role in Craniofacial encompasses the following components: Outpatient clinical care, including routine travel to outreach clinic sites Inpatient patient management and care coordination (with a team of advanced practice providers) Weekend rounding and call (shared with team) 1 st call coverage Monday-Friday, 8am-5pm Provide coverage during colleagues' LOA, FMLA, vacation, PTO, EDU, etc.

Requirements

  • Professional advanced practice provider licensure as Nurse Practitioner or Physician Assistant:
  • Nurse Practitioner: Current RN license and Advanced Practice Registered Nurse license through the Minnesota Board of Nursing Completion of a (pediatric or family) nurse practitioner program with a Master of Nursing Degree or a Doctorate of Nursing Practice Current national certification from one of the following: American Academy of Nurse Practitioners American Nurses Credentialing Center Pediatric Nursing Certification Board
  • Physician Assistant: Graduate of an accredited Physician Assistant Program and certified as a Physician Assistant by the National Commission on Certification of Physician Assistants (NCCPA) Licensure as a PA in Minnesota; certification and prescriptive authority
  • Current CPR- BLS for Healthcare Providers by the American Heart Association
  • Demonstrated interpersonal communication, organizational and leadership skills.
  • Ability to function autonomously as a provider.
  • Ability to drive to multiple locations, including Gillette outreach locations
  • Current CPR certification through American Heart Association, or within 90 days of hire

Nice To Haves

  • Two years relevant clinical experience
  • Experience with management of surgical patients
  • Experience with pediatric patients, patients with craniofacial issues, or both
  • Demonstrated competence in working with individuals with disabilities and their families.

Responsibilities

  • Provides patient care – Creates individualized plans of care that are evidence-based. Successfully implements interventions. Effectively prioritizes. Evaluates patient care based on desired outcomes. Communicates patient-related data and information to enhance active and informed decision making by patients, families, nurse colleagues, and other healthcare providers. Provides patient care across the continuum including outpatient, inpatient and the perioperative areas
  • Performs specialized procedures after additional training and demonstrating competency, including: Provide inpatient care (admission through discharge) Perform history and physical examinations Interpret and evaluate patient data Order or perform diagnostic procedures Order or perform therapeutic procedures Provide instructions regarding patient care, disease prevention, and health promotion (eg. interventions to improve feeding in newborns and infants with cleft lip and palate) Create and maintain appropriate records Prescribe, administer, and dispense drugs, controlled substances, and medical devices in accordance with the Delegation Agreement Surgical Wound assessment and management Make referrals to other providers as appropriate
  • Family centered care - Provides patient care and health education reflecting specific patient, family, cultural and community needs. Identifies the learning needs of the population served. Implements an effective teaching plan. Employs strategies to promote health in a safe environment. Seeks opportunity to evaluate effectiveness of health education as culturally and developmentally appropriate.
  • Procedures and Policies – Identifies, interprets, and implements organizational policies and procedures according to regulations, professional and community standards. Protects the confidentiality of personal health information.
  • Active Engagement – Actively represents the department in committees, task forces, unit activities or other work groups and disseminates information and decisions to colleagues and peers using the Engaged Professional Practice Model. Ensures accountability through the use of dashboards and promotes transparency through reporting of near misses and medical errors. Attends staff and department meetings to the standard established for the department. Demonstrates meaningful contributions to the productivity of the professional environment of the department. Supports work of Collaborative Governance initiatives.
  • Participates in activities that promote professional growth, ease of access, quality improvement and patient satisfaction.
