JOB SUMMARY - Outpatient Cardiovascular Center The Advanced Practice Provider (APP) provides general and specialty medical and/or surgical services to individuals, families and groups, emphasizing health promotion and disease prevention as well as diagnosis and management of acute and chronic diseases. The APP will have an opportunity to practice autonomously and collaboratively within their established scope. All TGH team members are responsible for performing job duties in accordance with the mission, vision and values in accordance with Tampa General Hospital. Essential Functions-Cardiovascular Center Specific · Completes admissions for CVC patients, including comprehensive histories, physical examinations, and initiation of appropriate admission orders. · Manages patient flow for individuals presenting for outpatient cardiovascular procedures across Interventional Cardiology, Electrophysiology, Structural Heart, Heart Failure, and Vascular Surgery. · Prepares patients for procedures by ordering and reviewing required pre ‑ procedure testing to ensure clinical readiness and safety. · Coordinates safe and timely discharges for same ‑ day and short ‑ stay patients, including post ‑ procedure instructions, follow ‑ up planning, and communication with referring providers. · Monitors patients throughout their stay in the CVC, addressing changes in condition and ensuring continuity of care between procedural teams. · Ensures documentation meets requirements for cardiovascular registries, accreditation programs, and specialty ‑ specific reporting standards. · Collaborates with procedural teams to optimize scheduling, throughput, and patient experience within the CVC environment. · Serves as a clinical resource for patients and staff regarding outpatient cardiovascular procedures, preparation, and recovery expectations. · Serves as a clinical resource for patients and staff regarding outpatient cardiovascular procedures, preparation, and recovery expectations. Essential Functions-Advanced Practice Provider · Works autonomously as well as in collaboration with interdisciplinary teams to assess, diagnose and manage patient's health care concerns. · Maintains all required documents with supervising physician, medical staff and TGH, as required by state professional regulatory agencies. · Coordinates patient care in the inpatient and/or ambulatory setting, understanding and utilizing the available resources, to provide world class patient care. · Participates in departmental and organizational mandatory meetings and trainings, as well as performance improvement activities, such as the development and revision of standards, protocols or critical pathways. · Performs duties according to established procedures, protocols, professional standards, and evidence-based guidelines. · Requests, interprets and evaluates diagnostic and laboratory tests and takes appropriate actions as medically indicated in accordance with standards of care. · Identifies and escalates any specific signs and symptoms, problems or potential problems requiring further assessment, communication and/or intervention to promote positive outcomes. · Flexibility in meeting the needs and ensuring the well-being of those patients under our care. · Adaptability to the clinical service needs. · Involvement with quality and safety initiatives in accordance with high reliability standards. · Promotes education of other professionals in developing skills and knowledge related to providing optimum health care. · Demonstrates a commitment to compassion, care, inclusion, and integrity in all interactions with patients, families, and team members. · Provides education to patients and their families, as well as educational outreach to the community. · Demonstrates initiative and leadership qualities in interactions with teams, assigned project and/or groups.
