About The Position

Hill Country Health and Wellness Center is a Federally Qualified Health Center providing integrated primary care, mental health care, substance use disorder treatment, dental care and other community programs and services aimed at improving the health and well-being of people living in our community. We are a mission-driven agency that was started locally in the 1980s by a small group of young friends with a simple commitment to helping people in their community live healthier, more fulfilling lives. Over the years these efforts have led to the development of a variety of direct services and programs. Hill Country has two main clinics in Round Mountain and Redding. In addition, we provide urgent mental health services that include a field-based Mobile Crisis Outreach Team. Our Wellness Centers in Round Mountain and Burney serve adults and teens. We also have several "complex care" teams working in Redding and in eastern Shasta County. These teams include clinicians, case managers, and nurses who work with primary care and psychiatry to provide intensive outpatient care for people with chronic health conditions, serious mental health problems, substance use issues, and other life challenges. POSITION SUMMARY: Under limited supervision, the APP will deliver patient-centered primary care services, including appropriate assessment, diagnosing, treatment and management of patient's healthcare needs across the lifespan, with an emphasis on providing care to underserved populations focused on preventative healthcare, chronic disease management and patient education. APP will work as an integral part of HCCC integrative health care team by participating in regular meetings and quality improvement.

Requirements

  • Master's degree in Nursing or in Physician Assistant Studies from an accredited program.
  • Current certification through a nationally accredited Physician Assistant or Nurse Practitioner program.
  • Active and unrestricted NP / PA California license.
  • Current BLS / ACLS certification
  • Current drivers' license
  • DEA License
  • Strong clinical skills (ability to diagnose, treat, and manage patient care); intermediate language skills (ability to read, write and interpret routine documents); basic math skills (ability to perform basic math required for patient care / medication management); clinical judgement / reasoning (ability to perform high level reasoning in different clinical situations).
  • Must have good working knowledge of standard medical equipment.
  • Excellent interpersonal and communicate skills to work with diverse populations.
  • Proficiency in EHR systems and basic working knowledge of Windows operating systems (Word, Excel, Outlook and internet browsers).

Nice To Haves

  • Prefer at least one year experience in a clinical setting.
  • Experience in rural community clinic preferred.
  • Strong leadership skills beneficial.

Responsibilities

  • Provide high-quality, cost-effective, comprehensive primary care services to patients of all ages utilizing an integrative team-based approach to meet the needs of each patient.
  • Utilize patient-centered care that considers social determinants of health, mental health, cultural sensitivity and holistic approaches to wellness.
  • Perform patient assessments, including history taking, physical examinations, and developing diagnostic and therapeutic treatment care plans, including prescribing medications and appropriate specialist referrals, according to scope of practice guidelines.
  • Document patient care in Electronic Health Records (EHR) system, ensuring accurate, thorough and timely entries.
  • Ensure proper follow-up care including timely review of all labs, diagnostic imaging, referrals, and specialty consults.
  • Promote wellness and disease prevention through patient education, screenings and counseling.
  • Conduct initial health assessments, well-child visits, immunizations, men / women's health examinations, and annual wellness visits in compliance with preventative care guidelines.
  • Obtain consultation with assigned supervising physician when appropriate according to the practice agreement / standardized procedures.
  • Collaborate with an integrative health care team through regular participation in medical staff meetings, peer review, case conferences and quality improvement initiatives.
  • Willingness to perform other duties within the scope of practice as assigned.
