About The Position

The Advanced Practice Provider (APP) at Almost Home Kids works within a multidisciplinary clinical team to deliver individualized patient care, including assessment, diagnosis, and management of health issues. This role emphasizes evidence-based practice and includes participation in research and academic activities. The APP will rotate between Chicago and Naperville locations and is supported by a fellowship program designed to facilitate the transition from academic to clinical practice for new graduate APPs.

Requirements

  • Master's degree or higher from an accredited school appropriate for certification as a Nurse Practitioner or Physician Assistant.
  • Two (2) years of relevant clinical experience is preferred.
  • Licensure in the State of Illinois as an APRN/PA is required.
  • Clinical privileges through credentialing by the Medical Staff Office achieved within 90 days of hire.
  • Basic Life Support certification required.
  • Pediatric Advanced Life Support certification and/or other specialty certification is required in some areas within 6 months of employment.
  • Demonstrates excellent interpersonal skills, basic computer skills, and strong verbal and written communication skills.

Nice To Haves

  • Experience in pediatric care is preferred.

Responsibilities

  • Provides expert clinical care, education, consultation, advocacy, research, leadership, and mentorship.
  • Manages primary, acute, and chronic health problems through comprehensive history and physical examination assessment and diagnosis, including palliative and end-of-life care.
  • Orders, performs, and interprets diagnostic and therapeutic tests and procedures.
  • Orders and prescribes pharmacologic and non-pharmacologic interventions and performs other therapeutic or corrective measures to treat illness and improve health status.
  • Promotes the advancement of the profession at Lurie Children's and in the community, including publication and participation in professional organizations.
  • Collaborates and coordinates care with the medical team to deliver evidence-based care and establish best practices in clinical care.
  • Participates in research design, implementation, and interpretation.
  • Meets the Medical Staff's Allied Health Practitioners core measures of performance related to patient care, medical knowledge, practice-based learning and improvement, interpersonal communication skills, professionalism, and system-based practice.
  • Adheres to service principles with a customer service focus of innovation, service excellence, and teamwork.
