About The Position

Advocate Condell Medical Center is currently seeking a qualified Advanced Practice Provider (APP) to join our team! Condell Medical Center is a nationally renowned acute care hospital that provides a comprehensive range of medical services and is home to Lake County's first and only Level I trauma Center. Using a very hands-on approach, the APP will provide services to the acutely injured trauma patient across the care continuum, including initial trauma activation response, assessments, admissions, procedures, education, and discharge planning, all while collaborating with a team of board-certified surgeons.

Requirements

  • Master's Degree or equivalent as a Physician Assistant effective January 1, 2018 & 1 year PA experience in the relevant setting preferred.
  • Master's Degree or equivalent as a Nurse Practitioner & 1st Assist Certificate.
  • Well-developed interpersonal and communication skills.
  • Proficiency with an Electronic Medical Record.
  • Current licensure in the State of Illinois.
  • Current Illinois Controlled Substance License within 6 months of hire.
  • Eligible for DEA registration; Possession of a current DEA Registration within six months of hire and may vary based on practice site.
  • Current BLS Certification.
  • Current PALS/ACLS certification may vary based on practice site.
  • ATLS certification required within 1 year of start date.

Responsibilities

  • Respond and assist in the care of level 1 & 2 trauma activations.
  • Manage care of in-house trauma patients across the continuum.
  • Perform critical care procedures including but not limited to chest tube and central line placement.
  • Fulfill billing and productivity metrics requirements where appropriate and maintain relevant documentation.
  • Complete and document accurate comprehensive, episodic history and physical assessments.
  • Order, conduct, and interpret diagnostic laboratory and imaging studies.
  • Formulate a comprehensive plan of care in collaboration with the patient and interdisciplinary team.
  • Advocate for patients, families, co-workers, the community, and the Advanced Practice Practitioner profession.
  • Participate in peer review in accordance with privileges, competency assessment activities and maintain appropriate credentialing and privileging requirements.
  • Seek experiences and formal and independent learning activities to maintain and develop clinical and professional skills and knowledge.
  • Establish an effective relationship with the interdisciplinary team and the collaborating physician.
  • Advance the profession through presentations and publications.
  • Provide education to patients, families, significant others, nursing units, and the surrounding community using established education materials.
  • Encourage involvement in professional organizations.
  • Provide feedback to and consult with leadership in assessment, problem-solving, decision-making, and evaluating system issues and models of care.
  • Identify and participate in quality-improvement initiatives within the practice setting.
  • Participate in multidisciplinary teams to provide integrated delivery of patient care services.
  • Initiate and revise protocols or guidelines and facilitate organizational change to reflect evidence-based best practice relative to practice setting.
  • Work to influence policy-making bodies to improve patient care.
