About The Position

Will initiate, modify and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy. Focus on management of complex disease states to help patients reach optimal therapeutic goals. Simplify patient medication regimens, minimize the risk of harmful medication interactions and educate patients to enhance adherence and clinical outcomes. Responsible for policy development, Continuous process improvement, continuing education, clinical and medical staff development and Pharmacy APPE student and resident preceptorship. Performs patient care assessments, orders and interprets laboratory tests, prescribes, modifies and discontinues medication therapies. Monitors clinical outcomes, communicates with medical providers, performs electronic documentation and refers patients to other health care providers.

Requirements

  • Graduate of an accredited school of pharmacy in the U.S.
  • 4 years recent relevant experience, preferably in oncology pharmacy.
  • Registered Pharmacist (PHARMR) license upon hire.
  • ASHP-Post Graduate Year 1 General Pharmacy Residency upon hire.
  • ASHP-Post Graduate Year 2 General Pharmacy Residency in Oncology preferred.
  • Hospital Medical staff Credentialing required within 6 months post-hire.

Nice To Haves

  • Advanced Practice Pharmacist (APh) license preferred.
