About the position

This Advanced Practice Nurse position is in Primary Care Service at the VA Providence Healthcare System, located at the Middletown location. This position is full-time at 40 hours per week. The role includes independently managing a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. The nurse will develop and implement an individualized plan to achieve optimal population health and disease management, demonstrate leadership, and work collaboratively with the Primary Care Team and other interdisciplinary team members. Responsibilities also include ordering and interpreting labs, tests, and diagnostic studies, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals and consults to specialty services, providing education to patients and their families, and assessing and adjusting the plan of care as needed to achieve optimal patient outcomes.

Responsibilities

Requirements

  • U.S. Citizenship.
  • Active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
  • Basic Certification in an area of clinical specialty from a recognized certifying body.
  • Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner from an accredited institution.
  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program.
  • Participation in the COVID-19 vaccination program.

Nice-to-haves

  • Primary Care experience preferred.
  • 1 year of Clinical Experience.
