About The Position

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. Duties and responsibilities include, but are not limited to, the following: Independently manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develop and implement an individualized plan to achieve optimal population health and disease management. Demonstrate leadership and works collaboratively with Primary Care Team and other interdisciplinary team members. Order and interpret labs, tests, and diagnostic studies. Prescribe pharmacologic and non-pharmacologic interventions. Initiate referrals and consults to specialty services. Provide education to patients and their families.
