About The Position

The APN/NP II position reflects the mission, vision, and values of Northwestern Medicine, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. The Advanced Practice Nurse/Nurse Practitioner (APN/NP) works within the guidelines of a collaborative practice agreement with an active member of the medical staff of Central DuPage Hospital. In partnership with physicians and other health care team members, the APN/NP assesses, plans, implements, coordinates, and evaluates care for a specific patient population based on individual scope of practice. Through the application of evidenced-based decision-making, the APN/NP serves as a leader, educator, researcher and clinical mentor to all members of the health care team with the goal of achieving optimal patient outcomes. The APN/NP practices utilizing the scope and standards of practice for the advanced practice nurse as published by the American Nurses' Association.

Requirements

  • At least 5 years of experience in area of specialty
  • Masters of Science in Nursing
  • Registered Nurse in Illinois
  • Licensed Advanced Practice Nurse
  • Ability to read, write and communicate effectively in English
  • BLS certified

  • Certification as BLS, PALS, or ACLS instructor as needed

Responsibilities

  • Uses organizational and unit/department resources efficiently.
  • Manages work schedule efficiently, completing tasks and assignments on time.

  • Participates in efforts to reduce costs, streamline work processes, improve, and grow services we provide.
  • Collects and synthesizes comprehensive patient data based on utilization of evidence-based assessment techniques and instruments including but not limited to health and medical history, physical examination, interpretation of diagnostic tests and procedures, and assessment of preventive health screening participation.
  • Systematically compares, and contrasts clinical findings with normal and abnormal variations in formulating an appropriate differential diagnosis based on the integration of complex assessment data.
  • Identifies expected outcomes that promote cost effectiveness, patient satisfaction and continuity amongst providers, which incorporate scientific evidence and are achievable through the implementation of evidence-based practice.
  • Develops and implements an appropriate treatment plan that reflects current evidence and expert clinical knowledge and prioritizes interventions by ordering appropriate diagnostic tests, identifying appropriate pharmacologic and non-pharmacologic agents, developing a patient education plan, and incorporating needs of the individual, family, and community.
  • Supports collaboration with nursing colleagues and other disciplines to implement the plan of care.
  • Uses prescriptive authority, procedures, referrals, treatments, and therapies in accordance with state and federal law and regulations.
  • Evaluates patient progress toward expected outcomes and modifies plan of care appropriately.
  • Updates physician when acute changes in patient condition warrant immediate physician intervention.
  • Documents all pertinent data in the medical record.
  • Participates in the identification, development, and implementation of innovative quality improvment initiatives to address patient care challenges while maintaining a balance of cost effectiveness and quality of care.
  • Utilizes current research findings to expand clinical knowledge and enhance role performance.
  • Engages in a formal process seeking feedback regarding one's own practice from patients, peers, professional colleagues, and others as identified.
  • Serves as teacher/coach/mentor to front-line staff members performing patient management activities.
  • Integrates ethical considerations into all areas of practice by discussing risks, benefits, and outcomes of healthcare interventions.
  • Promotes advancement of the profession through writing, publishing, and group presentations both internally and externally.
  • Disseminates research findings to front-line staff and participates in nursing research activities in partnership with CDH nursing staff.
  • Initiates and/or revises protocols or practice guidelines to reflect current evidenced-based practice.
  • Performs other duties as assigned
