Position Overview We're seeking a compassionate and skilled Primary Care Advanced Practice Provider to deliver comprehensive medical care to adults of all ages (and children, if family medicine trained). You'll work as part of an integrated care team serving medically underserved individuals in our community, many of whom have mental health conditions and substance use disorders. This role qualifies for HRSA National Health Service Corps (NHSC) scholarship and loan repayment programs. What Makes This Role Unique Integrated Care Model: Collaborate with psychiatry providers, counselors, case managers, addiction medicine specialists, harm reduction team, dental team, and clinical pharmacists Diverse Practice Opportunities: Addiction medicine Infectious disease management (STI screening/treatment, HIV PrEP/treatment, Hepatitis C treatment) Sexual and reproductive health services (LARCs, colposcopy) Common primary care procedures Mission-Driven Work: Serve a diverse patient population with comprehensive, compassionate care

Requirements

  • For Nurse Practitioners: Master's or Doctoral degree in Nursing with valid Ohio license
  • Current, full, active, unrestricted license as Adult Nurse Practitioner or Family Nurse Practitioner
  • Current certification from the American Nurses Association or nationally recognized certifying body
  • Ability to obtain valid DEA license
  • Work within a Collaborative Care Agreement with supervising physician
  • For Physician Assistants: Graduate of ARC-PA accredited PA training program
  • Master's degree from accredited PA program or health sciences related field
  • Current NCCPA certification (PANCE) OR recent graduate eligible to take PANCE
  • Ability to obtain valid DEA license
  • Work within a Physician Assistant Supervision/Collaborative Practice Agreement

Responsibilities

  • Provide evidence-based primary medical care including chronic disease management, preventative care, and acute medical issues
  • Counsel and educate patients regarding health maintenance, disease prevention, diagnosis, treatment, and follow-up care
  • Complete accurate, timely, and comprehensive documentation in patient records
  • Participate in shared call system for 24-hour patient coverage (rotates among providers)
  • Collaborate with integrated care team to establish treatment/service plans
  • Provide non-discriminatory care in accordance with federal and state grant requirements
  • May travel between sites as necessary
