What you’ll do: Loyola University Medical Center is seeking a skilled and compassionate Advanced Practice Nurse (APN) to join our Heart Failure Program. This role provides advanced nursing care in an acute care setting, focusing on a specialized patient population. The APN will work both independently and collaboratively with physicians, case managers, and the healthcare team to ensure comprehensive, high-quality care. Key Responsibilities: Deliver expert care to heart failure patients, promoting healing and preventing illness and hospital readmission. Assess patients’ health status through comprehensive history-taking, physical exams, and psychosocial evaluations. Develop, implement, and modify individualized treatment plans based on patient needs and clinical data. Perform specialized procedures and interventions within the scope of practice. Coordinate care transitions across service units to optimize patient outcomes. Educate patients and families, empowering them to participate in their healthcare decisions. Maintain high standards of practice in alignment with Loyola University Medical Center’s mission and care models.
