About The Position

As a Provider at Lucet, you’ll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care. Please note the position requires you to meet patients in their homes throughout Erie County/Niagara County, NY and you will be expected to cover a territory radius of 50 miles from your home address.

Requirements

Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.
Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant
Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements.
Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.
MVR with reliable transportation and clean driving record.
DEA license and BLS certification
Must be a self-motivated professional who enjoys working independently and making a meaningful impact on member health.
Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
Ability to document assessment findings accurately and thoroughly
Maintain patient, team member and employer confidentiality while remaining in compliance with all HIPAA regulations.
Demonstrated clinical skills necessary to perform in-home assessments
High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
Frequent use of computer and phone systems
Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations
Ability to lift and carry up to 20 lb

Nice To Haves

Preferred two years of experience in a primary care or longitudinal care setting.

Responsibilities

Conduct in-home visits (minimum 25/week), including assessments, exams, and medication reviews.
Manage chronic conditions and develop individualized treatment plans.
Maintain accurate EMR documentation and comply with healthcare regulations.
Educate patients on disease management and healthy living.
Collaborate with interdisciplinary teams to ensure coordinated care.
