This recruitment will be used to fill Per Diem Advanced Practice Clinician positions in our health clinics throughout the Portland Metro Area.

Requirements

  • Master's Degree in Nursing (NP or PA).
  • Three (3) to six (6) years of experience that demonstrates the ability to perform the duties of the position.
  • License required to practice medicine as a Nurse Practitioner or Physician Assistant as outlined by Oregon Revised Statutes (ORS) 678.375.
  • CPR certification required.

Responsibilities

  • Diagnosing, managing and treating acute and chronic medication conditions including prenatal care, according to the scope of practice.
  • Conducting a medical history, physical exams, ordering and interpreting diagnostic tests, devising a course of treatment, discussing alternative treatment, risk and benefits of treatment.
  • Prescribing and monitoring medications, appropriate counseling of patient and reassessment of the patient at appropriate intervals.
  • Utilizing panel based information technology tools to prioritize and direct proactive care for patients assigned to the provider panel, including preventive health screening and chronic disease care management.
  • Providing follow-ups on utilization issues (ED, hospital admissions).
  • Establishing need for specialty and rehabilitative services and making appropriate referrals.
  • Assessing and counseling patients regarding preventive care needs; delivering services or making appropriate referrals.
  • Determining need/scheduling for high risk referrals.
  • Seeking clinical consultation from other providers as needed.
  • Providing consultation to nurses and support staff.
  • Prioritizing and directing activities of the care team.
  • Coordinating with Behavioral Health providers to effectively integrate care.
  • Providing coverage for clinical colleagues in their absence.
  • Completing patient documentation and managing electronic in-basket according to established policies and procedures.
  • Participating in quality improvement and assurance projects as needed.
