About The Position

THIS WORK MATTERS! Integrated Clinical Services provides quality health services for people who experience barriers to accessing care. The division operates medical and dental clinics, outreach programs, and provides health services in the county's jails and juvenile detention facility. As an Advanced Practice Clinician you will provide professional, comprehensive specialized nursing services with an emphasis on prevention of disease and promotion of health. The Advanced Practice Clinician establishes a therapeutic relationship with a diversity of patients, including refugees and low-income populations. Knowledge, Skills and Abilities (KSA): Bilingual in Spanish and English (language testing may be required). The Advanced Practice Clinician provides professional, comprehensive specialized nursing services with an emphasis on prevention of disease and promotion of health. They establish a therapeutic relationship with a diversity of patients, including refugees and low-income population groups and create a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community. Requirements

  • Masters degree or equivalent in nursing from an accredited college or university
  • Possession of a valid Nurse Practitioner certificate
  • License to practice as a Registered Nurse in the State of Oregon
  • CPR/BLS certification
  • DEA certification

Responsibilities

  • Perform comprehensive health assessments and clinical procedures by completing a physical and psychosocial history; identify actual and potential health needs based on medical, behavioral, financial, and social factors; prescribe and dispense medications to patients; initiate and/or continue medical regimes for new patients.
  • Examine, treat, and advise patients on medical and behavioral health problems; make referrals to appropriate social or medical agencies; chart treatment records and monitor results.
  • Act as representative on committees, interagency task forces, special projects, and other community outreach activities as assigned; provide consultation and education to internal and external partners and providers.
  • Analyze and review federal, state, and local laws, regulations, policies, and procedures in order to ensure compliance; conduct analysis on best practices and trends, and formulate recommendations.
