About The Position

Ascension Illinois is looking for an experienced Board Certified Nurse Practitioner or Physician Assistant for our Internal Medicine Department. This is a great opportunity for the right candidate to partner with Ascension Illinois to provide medical services to patients. Position Highlights: 100% outpatient Provide primary care, population health, and immediate care-type services to Unite Here union members (hospitality workers and their adult dependents) Monday-Friday 8:00am - 6:00pm and Saturday 8am - 12pm Saturdays and potentially Fridays are rotating See 10-20 patients per day on average in the clinic setting Nice mix of patients and cases A lot of autonomy Spacious exam rooms Busy practice Pharmacy on site Excellent office support staff Located in downtown Chicago Great city location Bilingual preferred (Spanish, Mandarin or Cantonese, etc) About Ascension Illinois Ascension Illinois is an award-winning health system committed to delivering outstanding, compassionate care to communities we are privileged to serve in Chicago and the surrounding region. Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
  • Nurse Practitioner credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date. Credentials are verified by local staff credentialing office

Nice To Haves

  • Drug Enforcement Admin credentialed from the Drug Enforcement Administration.
  • Registered Nurse.
  • Bilingual preferred (Spanish, Mandarin or Cantonese, etc)

Responsibilities

  • Obtain patient history and performs assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunizations and patient care.
  • Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
  • Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources.
  • Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.
