Under the direction of Ambulatory Clinical Pharmacy Services, the Advanced Pharmacy Practice Provider (APPP) provides oversight and leadership for appropriate medication use within their region of the health system. The APPP works closely with the medical and administrative leadership as well as the primary care providers to develop and implement strategies which continuously improve the quality of patient care at the best value for the patient and our health system. The APPP serves as an essential member of the ambulatory care team to achieve and maintain improved patient health outcomes, enhanced patient experience, reduced health care costs, and improved provider work life balance. The APPP practices at the top of licensure and with expanded scope of practice through a collaborative practice agreement (CPA). The APPP supports population health strategies and value-based care initiatives. The APPP coordinates care with the provider, care team, and other pharmacy services across Henry Ford Health. The APPP participates in ongoing research, scholarship, quality initiatives, projects, committees, policy development, and formulary management strategies. The APPP assists in identifying opportunities to streamline patient care, improve the provision of pharmacy services, decrease waste, and rework in the region, and improve drug utilization. The APPP will be instrumental in guiding the further development of other pharmacists on the health care team.

  • Provide oversight and leadership for appropriate medication use within the health system region.

  • Serve as an essential member of the ambulatory care team.

  • Coordinate care with providers, care teams, and pharmacy services.
  • Participate in research, quality initiatives, and policy development.
  • Identify opportunities to streamline patient care and improve pharmacy services.
  • Guide the development of other pharmacists on the health care team.
  • Doctor of Pharmacy degree or equivalent experience.
  • PGY1 Pharmacy Practice Residency and PGY2 Ambulatory Care/Primary Care Specialty Residency.
  • 3-5 years of top-of-license clinical experience in ambulatory care or applicable specialty area.
  • Prior experience and knowledge of integrated health systems.
  • Experience working with relational databases.
  • Knowledge of Pharmacoeconomics and biostatistical applications in drug therapy decision making.
  • Exemplary oral and written communication skills.
  • Ability to lead work groups effectively.
  • Board Certified Ambulatory Care Pharmacist (BCACP) or eligible to take the exam within 1 year.
  • Board Certified Pharmacotherapy Specialist (BCPS) or Board Certification in applicable specialty area.
