Experienced and highly trained pharmacist who works in a specialized area performing high level functions (e.g., Oncology/Infusion Center, Pediatrics/Neonates, Decentralized Roles e.g., Unit Based Emergency Department Pharmacist) Works in 1 or more specialty areas greater than 50% of the time Specialty areas include but not limited to: Oncology/Infusion Center, Pediatrics/Neonates, Decentralized Roles (e.g., unit-based ED pharmacist) Proactively contributes to the decision-making process and the development of patient-centered care plans Deliver satisfactory and valued services and finding mechanisms to reduce medication costs Participate in quality, safety and other project work related to the specialized area Assists in the development of policy and procedures Unit Based Obtains a patient’s medication history using multiple sources of validation Provides patient education using teach back method Conducts antimicrobial stewardship activities Reviews treatment appropriateness of urinary tract infections Attends codes and bedside medication emergencies Rounds with the team, if applicable
