About The Position

The position focuses on specialized areas including Oncology/Infusion Center, Pediatrics/Neonates, and Decentralized Roles such as unit-based ED pharmacist. The role involves proactive contributions to decision-making processes and the development of patient-centered care plans. The pharmacist will find mechanisms to optimize medication usage and participate in quality, safety, and other project work related to their specialized area. Additionally, they will assist in the development of policies and procedures relevant to their specialty.

Requirements

• Graduation from an accredited School of Pharmacy with PharmD required.
• Current active registration and licensure with the State of New Jersey Board of Pharmacy required.
• More than 1 year of experience in a specialized area or a PGY-1 Pharmacy Residency or a PGY-2 Pharmacy Residency in a specialized area.

Responsibilities

• Reviews and monitors patient plans for appropriateness based on clinical and institutional guidelines.
• Identifies treatment issues and medication misuse.
• Assesses medication orders to ensure compliance with established standards and practice guidelines.
• Advises clinicians on drug selection based on clinical guidelines and patient-specific considerations.
• Adheres to safety requirements in handling and preparing chemotherapy and hazardous agents.
• Participates in educational activities and answers drug information questions.
• Trains, educates, and mentors other pharmacists and technicians, and precepts pharmacy students and residents.
• Participates in quality, safety, and project work related to oncology services in the outpatient infusion setting.
