Position Objective: Advanced Medical Assistants (AMA) perform a variety of administrative and clinical tasks to help improve care to patients, and support providers. They are an essential part of the advanced primary care team and facilitate meeting the goals of the Maryland Primary Care Program. Their main responsibility is to oversee the Remote patient monitoring program and the support the physician in Telemedicine on Demand program They help the physician determine how best to meet patient needs and engage them in their own care using a variety of tools, processes, and personnel as appropriate. They maintain knowledge of care management, behavioral health, and chronic disease self-management resources available to patients and facilitates patients' adoption of these services. They play an integral role in promoting access to appropriate care, including overseeing care management and coordination activities for the patients of the primary care practice. Essential Job Duties: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions Pre-visit planning workflow to ensure care completion prior to visit whenever possible, and After-visit summary review with patients whenever appropriate Ensures patient engagement and education. Engagement by involving the patients in activities to improve their health. Education by cultivating the patient about self-management practices they can undertake to gain greater control of their health status. Manage many aspects of the patients care: referrals to specialists, hospitalizations, ER visits, ancillary testing, and other enabling services. Responsible for working with patient and patients care team to coordinate change readiness, needs assessment and develop an individualized treatment care plan Assist patients in setting SMART goals for self -management, teaching them how to do self-management tasks and report abnormal findings to their physician team. Assess barriers when patient has not met treatments goals, is not following treatment plan of care, or has not kept important appointments; Collaborate with physicians, hospitals, ER and other frequently used healthcare resources to clarify roles, develop effective timely communication between the care teams; and in assessing the patients progress toward individual health care goals. Oversees the development, procurement, and adoption of patient self-management educational resources used by the primary clinical teams.
