About The Position

The Advanced Illness Care Nurse provides comprehensive palliative nursing care to patients living with serious illnesses, focusing on symptom management, quality of life, and support for patients and their families. This role is responsible for assessing, planning, implementing, evaluating, and documenting individualized palliative nursing care in collaboration with the Interdisciplinary Group (IDG) and in alignment with the established plan of care and Clinical Services Department policies The Advanced Illness Care Nurse practices across multiple settings, including private homes and assisted living facilities, , and works closely with physicians and community providers to ensure coordinated, high quality, patient-centered care. This position supports Bluegrass Care Navigators’ mission by delivering compassionate, patient-centered care to all individuals served.

Requirements

  • Graduate of an accredited school of nursing.
  • Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or participating COMPACT state Board of Nursing.
  • Minimum of one (1) year of nursing experience.
  • Knowledge of symptom management, advanced directives, and end-of-life care principles.
  • Demonstrated clinical competence, sound nursing judgement, and excellent observational skills.
  • Strong organizational, planning, data management skills with the ability to manage multiple tasks simultaneously and possess the ability to manage and interpret data.
  • Ability to work independently, exercise discretion, and maintain professionalism under stress.
  • Skilled in providing education and feedback to clinical staff as appropriate.
  • Current CPR certification.
  • Ability to collaborate effectively within an interdisciplinary team (IDT).

Nice To Haves

  • Previous experience in palliative care or hospice nursing preferred.
  • Understanding of hospice and palliative care philosophy and principles preferred.
