We are seeking an Advanced Clinical Pharmacist Med Rec for our team to ensure that we continue to provide all patients with high quality, efficient care. The advanced clinical pharmacist shall practice the profession of pharmacy with the utmost code of ethics demonstrating the organization's mission and values. The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing medication therapy within an interdisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the care process.

Requirements

  • B.S. or PharmD. Degree in Pharmacy Required.
  • Maintains current pharmacist licensure Required.
  • National Provider Identifier (NPI) Required.
  • 3-5 years hospital pharmacy experience with supervisory experience Preferred.
  • Graduate of an accredited residency program Preferred.
  • (PGY1) Pharmacy Practice Residency Preferred.
  • (PGY2) Specialized Residency in area of specialty Preferred.

Responsibilities

  • Provides leadership in managing patient-centered medication reconciliation on admission and discharge.
  • Ensures medication history and reconciliation is completed according to established policies, procedures and protocols.
  • Continuously works with the pharmacy services team and interdisciplinary care team to improve medication history and reconciliation processes.
  • Provides training for and assures the continued competency of those involved in completing medication history and reconciliation.
  • Meets with the certified pharmacy technicians regularly to provide leadership and to share best practices to improve patient medication history safety.
  • Identifies and resolves drug related problems.
  • Verifies each medication documented by the Certified Pharmacy Technician/Medication History Technician in the electronic healthcare record.
  • Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols.
  • Communicates with providers to reconcile the admission medication history taking into consideration equivalent formulary alternatives.
  • Documents interventions in the electronic health record and in CPW/VigiLanz, and cosigns technician notes in the electronic health record.
  • Reconciles discharge medications, provides recommendations to the provider as necessary.
  • Performs discharge counseling for patients at risk of readmission.
  • Provides verbal and written medication education to enhance patient understanding.
  • Analyzes data to demonstrate the success of the pharmacy led medication reconciliation safety program.
  • Monitors key metrics to ensure program performance.
  • Documents good catches and medication events.
  • Precepts pharmacy students and PGY1/2 residents.
  • Provides education and training to pharmacy technician, pharmacist staff and the interdisciplinary healthcare team.
  • Verifies Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness.
  • Participates in all drug therapy management protocols.
  • Reviews/interprets culture and susceptibility data for antibiotic appropriateness.
  • Participates in the quality improvement and medication-use review activities of the department.
  • Supervises and directs pharmacy support personnel.
  • Maintains current pharmacist licensure, and maintains competence required for current job title/position.
  • Attends orientation, education, and training programs.
  • Provides courteous, cooperative service to patients, visitors, and staff.
