Frederick Health Medical Group (FHMG), a highly respected 160+ provider, multi-specialty medical group, is seeking an experienced Nurse Practitioner or Physician Assistant - to serve as the primary clinical navigator and care coordinator for patients undergoing genitourinary (GU), gastrointestinal (GI), and thoracic cancer surgery. This is a high-impact, relationship-driven role designed for an APP who thrives on continuity, coordination, and deep patient engagement. In this position, you are the single, trusted point of contact for each patient, guiding them seamlessly through pre-operative, peri-operative, and post-operative care within the Multidisciplinary Clinic (MDC) model.
