About The Position

Mon - Fri, no weekends or evenings. 6-8 patients per day, managing chronic conditions of your own patient panel.

Requirements

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver’s license and good driving record

Nice To Haves

• Geriatric training/experience preferred

Responsibilities

• The Advance Practice Provider (Nurse Practitioner or Physician Assistant) we seek are those who have directly related experience in home health, geriatrics, hospice and primary care.
• The Advance Practice Provider who wants to practice medicine with a focus on patient care, not volume.
• We want our clinicians to take the time needed to truly address the patient’s needs.
