Provides the highest quality direct patient care, medical direction and coordination of professional medical care within the practice in an efficient, cost effective, and accountable manner. May manage patients by providing primary health care and case management, focusing on disease prevention, wellness, and management of minor, chronic, and acute illness. Provides holistic care, taking into account the needs and strengths of the whole person. Collaborates with clients and other interdisciplinary health care members to establish realistic goals and activities to guide both the client and the provider in implementing care. This position is knowledgeable of the departments Quality Plan and population management program and actively supports the plan and program through daily activities including but not limited to education to other clinical support staff as needed.

Responsibilities

  • Performs medical evaluations and initial and follow-up injury/illness care; orders and interprets laboratory tests and examinations to determine treatment; orders treatment in conjunction with physician; follows the progress of the patients and makes changes in treatment as indicated; applies local anesthetic; examines and cleans wounds; incises and drains abscesses; sutures; makes appropriate legible records of findings during examinations to be reviewed and countersigned per department standards by the supervising physician; performs regular duties as delegated by the supervising physician.
  • Utilizes current “Best Practices” (TriHealth approved) to develop and implement plans, monitor Progress, and measure outcomes for patients (or, client company employees, or TriHealth employees) who are referred for Chronic Illnesses to include anticoagulation management.
  • Works with patients individually or in groups to promote and achieve effective symptom control and sustained attendance and productivity.
  • Coordinates activities with Primary Care Physicians and Specialists in the Community. May develop and present educational programs.
  • Promotes, supports, and engages in activities that will improve the overall health and wellbeing of patients.
  • Works with management and Collaborating Physician to ensure practice is consistent with medical policy and professional standards.
  • Cooperates and participates in the development, implementation and revision of policies affecting medical practice and the quality of care.
  • Participates in and assists with in-service and continuing education programs for patients, physicians, nurse practitioners, nurses, and clinical staff.
  • Makes recommendations for continued development of new and/or existing services within program or department.
  • Promotes, supports and engages in projects and activities which will develop or improve service delivery.
  • Attends required and recommended internal and external meetings.
  • Reviews reports relative to department/program/practice operations; participates in and assists with Quality Assurance processes; and demonstrates initiative in leading staff in quality of care issues.

  • Communicates requests for time off with management for approval.
