About The Position

Encompass Community Services is the largest provider of health and human services in Santa Cruz County. We have been delivering culturally appropriate and evidence-based safety net services to our community for nearly 50 years. We operate nearly 40 programs providing a wide range of services related to family and social well-being, early childhood education, behavioral health, housing and more. Encompass has a continuum of care for substance use disorder (SUD) treatment including residential treatment, withdrawal management, outpatient services, sober living, and Medication for Addiction Treatment (MAT) services. Located in the serene foothills of Watsonville, Si Se Puede is one of the only bilingual and monolingual SUD treatment environments in the nation. This residential program serves Spanish speaking, male clientele. The residential program is staffed by a highly skilled and trained team including nurses and certified behavioral health clinicians. We currently have an opening for an Advanced Practice Clinician (APC) (FNP or PA) at our SSP residential treatment program of 10 hours per week (with the option to increase up to 20 hours/week mid-summer 2026, if interested). Pay is dependent on experience ranging between $70-$85 per hour. Hours are flexible Monday-Friday between business hours. An additional weekend and holiday on-call stipend is available as part of the on-call rotation service. Services are provided in-person and via telehealth. Under the supervision of the Medical Director and Director of Nursing, the APC is responsible for providing medical care including withdrawal management, bridge primary care, MAT for OUD and AUD, and care coordination to SUD residents while collaborating with care teams delivering Integrated Behavioral Health treatment services.

Requirements

  • Unencumbered and unrestricted license to practice as a Family Nurse Practitioner (FNP) or Physician Associate (PA) within the state of California.
  • Unencumbered and unrestricted DEA licensure within the state of California.
  • At least 1 year experience as an APC.
  • Experience and comfort treating substance use disorders and prescribing MAT.
  • Current BLS for Healthcare Providers certification.
  • Proficient computer skills.
  • Ability to successfully navigate platforms such as Outlook, Excel, Teams, and EHRs.
  • Demonstrate skill in effective written and verbal communication
  • Ability to recognize the needs and concerns of diverse groups of people.
  • Demonstrate strong skills around attention to detail, prioritization and triage.
  • Demonstrate ability to take initiative and work independently with minimal supervision at the top of medical license.
  • Self-motivated and proactive in identifying and addressing challenges and in engaging with clients.
  • Demonstrate skills in effective written and verbal communication.
  • Ability to recognize the needs and concerns of diverse groups of people.
  • Must have a health examination including a screening for tuberculosis within 90 days prior to employment or within 7 days after employment and at least annually thereafter.
  • Must be able to pass a criminal record clearance.

Responsibilities

  • In collaboration with a team delivering integrated behavioral health treatment services, supports with care coordination through primary care establishment, access to dental services, referral coordination, medication reconciliation, and linkage to other resources.
  • Provide ongoing medical care including initial physical examinations within 72 hours of admission, bridge and follow-up care, withdrawal management, and treatment of substance use disorders.
  • Determine eligibility of client for SUD residential treatment during pre-admit and day of intake.
  • Provide support and direction to onsite nurses and medical assistants as needed.
  • Provide MAT services for AUD and OUD within scope of practice, including LAIs.
  • Order and review labs as clinically indicated.
  • Prescribe and monitor medications for clients in residential treatment.
  • Recognizes care priorities and potential crises and responds appropriately.
  • Assist clients in improving and maintaining stability by providing advocacy support and education.
  • Adheres to required infection control and aseptic techniques in the delivery of care.
  • Communicates closely with community healthcare providers to deliver integrated care and improve continuity.
  • Exhibits non-judgmental, empathetic and supportive approach when communicating.
  • Meets MediCal billing requirements through appropriate documentation on EHR.
  • Consistently uses clear, concise, and effective written and oral communication; complies with departmental policies when sharing/documenting relevant care data; promotes strict confidentiality and right to privacy.
  • Support with after-hours provider on-call rotation.
  • Perform additional duties as assigned.
