The role of a Case Manager is to work with adults (ages 21 and up) to identify needs and goals that will strengthen the adult's future. Areas of focus include but are not limited to mental health, physical health, economic needs, education and positive social development. In this position, the Case Manager provides ongoing support and guidance to clients on a weekly basis, both within the community and in their homes. Transportation may be required to facilitate essential connections to community resources and services. Collaborating with other service providers and professionals is essential to ensure a comprehensive and integrated approach to client care.
