The role of a Case Manager is to work with adults (ages 21 and up) to identify needs and goals that will strengthen the adult's future. Areas of focus include but are not limited to mental health, physical health, economic needs, education and positive social development. Clients are met with weekly on average and work towards these goals together in the community where the needs are as well as in the home itself. Staff transport clients when needed to make sure important connections to community resources are successful. The combined efforts of the case manager and the informed, client led approach, deliver some of the best outcomes in the field of human service work. It's challenging work for sure, but the outcomes and rewards of changing generational cycles and improving the quality of lives in our communities outshines everything else.
