About The Position

Under general supervision, the targeted case manager provides an intensive level of support that goes beyond the care coordination that is a basic expectation for all people served by the CCBHC. Adult targeted case managers provide supports for people aged 18 years and older who are at high risk of suicide or overdose, particularly during times of transition such as from residential treatment, hospital emergency department, or psychiatric hospitalization. Targeted case management should be available during other critical periods, such as episodes of homelessness or other transitions within the community as well as for individuals with complex or serious mental health or substance use conditions and for individuals who have a short-term need for support in a critical period, such as an acute episode or care transition. Targeted case management will be provided for other groups who are identified as being at elevated risk due to social factors, including but not limited to, those indicated by the Social Determinants of Health, racial inequities, and/or age. The adult targeted case manager performs comprehensive assessments to identify patient needs and assist them in gaining access to indicated medical, social, legal, educational, housing, vocational, and other services and supports with the goal of empowering consumers to regain control over their own lives while promoting the recovery process. The adult targeted case manager will make timely contact with patients who are discharged after receiving crisis services or treatment at local and state hospitals. Adult targeted case managers must continuously monitor patient progress toward treatment goals and intervene quickly as needs change. Extensive documentation is required to accurately capture services provided and patient progress. The adult targeted case manager will research and identify appropriate community resources, and refer, link, and coordinate with other service providers and community facilities. Develop relationships and act as liaison with community resources. Deliver education to patients and family members. Participate in development and implementation of shared care management protocols and referral processes. Follow up to assess and reassess patient’s utilization and benefit of the resources.

Requirements

  • Completion of a bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field.
  • One year of full-time experience in social services delivery.
  • Knowledge of mental illness required.
  • Must have a valid GA driver’s license.

Responsibilities
