About The Position

The Adult Rehabilitation Navigator Counselor at Good Shepherd Rehabilitation Network is a full-time role for a Licensed Clinical Social Worker (LCSW) who will provide psychotherapeutic treatment and educational support to patients with brain injuries, spinal cord injuries, and concussions. This position is integral to the outpatient multidisciplinary team, focusing on enhancing emotional well-being, promoting health and wellness, and facilitating psychosocial reintegration for patients and their families. The counselor will assess psychosocial needs, develop treatment plans, and maintain communication among the care team to ensure effective service delivery.

Requirements

• Master's Degree in Social Work required.
• Minimum of three years of experience in counseling patients in a medical or behavioral health setting.
• Licensed to practice in the State of Pennsylvania as a Clinical Social Worker.

Nice To Haves

• Experience in rehabilitation providing education and support to treatment teams.
• Medical experience implementing behavioral health interventions.

Responsibilities

• Assess psychosocial needs of patients and families/caregivers in collaboration with the treatment team.
• Conduct thorough reviews of medical records and obtain additional information from relevant sources.
• Coordinate efforts with other professionals and agencies to maximize client progress.
• Assist in formulating and administering treatment and behavior plans.
• Develop therapeutic intervention plans tailored to patient needs.
• Monitor patient progress and adapt treatment efforts as necessary.
• Interact with patients and families to assist in personal and social development.
• Provide regular updates to the treatment team and maintain confidentiality standards.
• Maintain current knowledge of practice and complete professional education activities.
• Accurately document evaluation, assessment, and treatment activities.
